Sample Collection
Patient must avoid mineral supplements, vitamin B12, and vitamin B complex for at least 3 days prior to specimen collection. If gadolinium or iodine-containing contrast media has been administered, wait at least 96 hours before collecting the specimen.
Method: Venipuncture with powderless gloves
Laboratory Analysis
Venipuncture must be performed using powderless gloves to prevent environmental cobalt contamination. A metal-free Royal Blue Top (K2EDTA) tube must be used for blood collection.
Report Delivery
Specimen should be shipped refrigerated. Do not freeze. Stability: Room temperature up to 6 hours; Refrigerated (2–8°C) up to 48 hours; Frozen samples are not accepted.

How to Prepare
- Avoid mineral supplements, vitamin B12, and vitamin B complex for 3 days before the test
- Wait at least 96 hours after receiving gadolinium or iodine-based contrast media
- Phlebotomist must use powderless gloves during specimen collection
- Collect exactly 4 mL (minimum 2 mL) of whole blood
- Use only the metal-free Royal Blue Top (K2EDTA) tube supplied by LPL
- Ship specimen refrigerated; do not freeze

"Cobalt exposure is an under-recognised cause of unexplained cardiomyopathy, neuropathy, and thyroid dysfunction. Patients with hip or knee metal-on-metal implants, industrial workers in hard-metal or battery manufacturing, and those on long-term vitamin B12 injections (cobalamin) should be periodically screened. Early detection through a simple blood test allows timely intervention before irreversible organ damage occurs. If you experience persistent fatigue, shortness of breath, hearing loss, or joint pain—particularly after implant surgery—discuss cobalt testing with your physician."

Blood cobalt level is within the normal reference range for the general population. No evidence of significant cobalt exposure.
Status: Normal
Range: < 1.0 µg/L
Cobalt levels are above normal. May indicate mild exposure or early accumulation. Clinical correlation recommended; repeat testing and monitoring may be warranted.
Status: Borderline / Elevated
Range: 1.0 – 5.0 µg/L
Significantly elevated cobalt level indicating potential cobalt toxicity. Urgent clinical evaluation is recommended. Patients with metal-on-metal implants should be assessed for implant failure. Further workup including cardiac, thyroid, and neurological evaluation may be indicated.
Status: Elevated / Toxic
Range: > 5.0 µg/L
Consult a physician immediately if your cobalt blood level is above 1.0 µg/L, especially if you have a metal-on-metal joint implant, work in an industry with cobalt exposure, or are experiencing symptoms such as shortness of breath, chest pain, fatigue, hearing loss, joint pain, cognitive difficulties, or irregular heartbeat.
