Dermatofibrosarcoma protuberans (DFSP) is a rare, slow-growing soft tissue sarcoma arising from the dermis. It accounts for approximately 1% of all soft tissue sarcomas. DFSP is characterized by a distinctive molecular signature: a chromosomal translocation t(17;22) that fuses the COL1A1 and PDGFB genes, leading to uncontrolled cell proliferation. Early recognition is critical because surgical excision with clear margins offers a high cure rate.
Etiology and Pathogenesis
DFSP originates from fibroblasts in the dermis. The hallmark genetic alteration is the t(17;22) translocation, which creates a COL1A1-PDGFB fusion gene. This fusion leads to constitutive activation of the PDGFB signaling pathway, driving tumor growth. While the exact cause remains unknown, risk factors include prior trauma, radiation exposure, and certain genetic predispositions. DFSP is not typically inherited, but somatic mutations are present in tumor tissue.
Early Signs and Symptom Progression
DFSP often presents as an asymptomatic, indurated plaque that gradually enlarges. Early lesions may mimic a scar, birthmark, or dermatofibroma. Over months to years, the plaque may become nodular and protuberant. The following symptoms warrant dermatologic evaluation:
- Unusual Skin Patch: Reddish-brown or pink patch resembling a scar or birthmark.
- Slow-Growing Lump: A firm, rubbery nodule develops within the patch.
- Irregular Borders: Edges may be uneven or jagged.
- Size Changes: Gradual enlargement over time.
- Skin Sensitivity: Tenderness, ulceration, or easy bleeding.

Treatment and Prognosis
The standard treatment is wide local excision with clear margins. Mohs micrographic surgery is increasingly used for optimal margin control. In cases where surgical margins are positive or tumor is unresectable, targeted therapy with imatinib (a PDGFB inhibitor) may be considered. With early detection and complete excision, the 5-year recurrence-free survival exceeds 95%. Regular follow-up is essential due to a small risk of late recurrence.
