Parosteal osteosarcoma most frequently involves the metaphyseal region of long bones, particularly the posterior aspect of the distal femur. Other common sites include:
- Proximal tibia
- Proximal humerus
- Fibula
- Rarely, flat bones such as the pelvis or scapula
The tumor typically grows slowly and may be present for months to years before diagnosis. Its low-grade nature means that metastasis is uncommon at presentation, but it can occur if left untreated.
Risk Stratification and Genetic Predisposition
While the exact etiology remains unclear, several risk factors have been identified:
- Age: Peak incidence occurs between 10 and 30 years, though it can occur in older adults.
