Who Should Get This Test
To assess immunological compatibility between donor and recipient prior to transplantation or blood transfusion by detecting preformed antibodies against donor T and B lymphocytes.
How to Prepare
- Collect recipient sample preferably 48 hours after dialysis.
- Recipient: Collect 6 mL whole blood in 1 Yellow Top ACD tube and 3 mL serum in 1 SST.
- Donor: Collect 12 mL whole blood in 2 Yellow Top ACD tubes.
- Ship refrigerated; do not freeze.
- Ensure mandatory doctor's prescription with stamp, 1 photograph each of recipient and donor, and Solid Organ Transplant Consent form Form 33 are provided.
Doctor's Notes
"For pre-transplant patients, a negative HLA T & B cell crossmatch is a key requirement before proceeding. Always collect samples as per protocol and provide mandatory consent, prescription, and photographs."
Test Parameters & Specifications
Sample Stability
- Frozen sample
- Inadequate sample volume
- Missing mandatory prescription, consent form, or photographs
- Improperly labeled specimen
Understanding Your Results
If the result is positive or if there is a history of prior transplant, blood transfusion, or pregnancy, discuss the report with a transplant nephrologist or immunologist.
Limitations
- ⚠CDC crossmatch may not detect very low levels of donor-specific antibodies
- ⚠B cell crossmatch may be positive due to non-HLA antibodies
- ⚠Does not replace DSA testing or clinical assessment
- ⚠Requires live donor lymphocytes; sample viability affects results
Risks & Considerations
- ●Mild pain or bruising at venipuncture site
- ●Slight bleeding or hematoma
- ●Light-headedness during blood draw
Interfering Factors
- ●Frozen or improperly stored specimens
- ●Incomplete clinical history or consent documentation
- ●Recent dialysis timing deviations
- ●Improper sample labeling
- ●Drugs affecting immune response may influence interpretation
