Sample Collection
Patient must strictly avoid L-Dopa and Levodopa/Carbidopa medications for at least 72 hours before starting the 24-hour urine collection. Inform the physician about all current medications, including antidepressants and MAO inhibitors. No other specific fasting is required, but dietary advice from the physician should be followed.
Method: 24-hour urine collection with acid preservation
Laboratory Analysis
Begin the 24-hour urine collection as instructed by the laboratory. Discard the first morning urine on day one, then collect all subsequent urine for the next 24 hours, including the first morning urine on day two. Add 50% hydrochloric acid (HCl) drop by drop to the collection container to bring the urine pH between 1 and 2. Do not use concentrated HCl. Keep the container refrigerated or on ice during collection.
Report Delivery
After completing the 24-hour collection, mix the urine well and transfer a 50 mL aliquot (minimum 10 mL) to the provided transport container. Ensure the sample is clearly labelled with patient name, date, and total urine volume. Ship the sample refrigerated or frozen to the laboratory. How to Prepare
- Use a clean, sterile 24-hour urine collection container provided by the laboratory
- Discard the first voided morning urine on day one; begin collecting from the next void
- Collect all urine passed over the next 24 hours into the container
- Include the first morning void on day two as the final specimen
- Add 50% HCl drop by drop to the container to bring urine pH between 1 and 2
- Never use concentrated hydrochloric acid for preservation
- Keep the collection container refrigerated or on ice throughout the collection period
- Record the total volume of urine collected over 24 hours

Elevated HVA may also guide the workup of pheochromocytoma and other neural crest-derived neoplasms. HVA levels are within the expected range for the patient's age. This suggests normal dopamine metabolism. However, a normal result does not completely exclude the presence of a catecholamine-secreting tumour, particularly if clinical suspicion remains high.

Slightly elevated HVA levels may be seen in conditions such as early-stage neuroblastoma, stress, medication effects (MAO inhibitors, L-Dopa withdrawal), or dietary factors. Markedly elevated HVA levels are strongly suggestive of catecholamine-secreting tumours such as neuroblastoma, pheochromocytoma, ganglioneuroblastoma, or other neural crest tumours. In patients under treatment for neuroblastoma, persistently elevated or rising HVA may indicate incomplete treatment response or disease recurrence. Urgent clinical correlation with imaging and specialist consultation is warranted.

Low urinary HVA levels may be observed in patients with Parkinson's disease or other conditions associated with reduced dopamine synthesis. Clinical correlation with neurological evaluation and other diagnostic tests is advised.
