"The Immunoglobulin IgD Serum Test is an important but specialized investigation. IgD constitutes less than 1% of total serum immunoglobulins, and its clinical significance is still being studied. This test is most valuable in evaluating suspected Hyper-IgD Syndrome (HIDS), certain cases of multiple myeloma, and selected immunodeficiency disorders. It is not a routine screening test and should be ordered only when clinically indicated based on the patient's symptoms, medical history, and other laboratory findings. Patients with recurrent fevers, unexplained lymphadenopathy, or elevated total serum proteins may benefit from this evaluation. IgD levels within the normal reference range suggest no significant IgD-related pathology. Clinical correlation is still advised if symptoms persist.

Markedly elevated IgD is characteristic of Hyper-IgD Syndrome (HIDS), a rare autosomal recessive auto-inflammatory disorder caused by mutations in the mevalonate kinase gene. Patients typically present with recurrent fever episodes, abdominal pain, joint pain, skin rashes, and swollen lymph nodes.
Range: Significantly above normal (>100 IU/mL, often >1000 IU/mL)
Very high serum IgD concentrations are found in a subset of multiple myeloma patients (IgD myeloma accounts for approximately 1-2% of all myeloma cases). Additional tests such as serum protein electrophoresis, urine Bence Jones proteins, and bone marrow biopsy are required for confirmation.
Range: Very high levels, often >1000 IU/mL
Mild elevations may be seen in chronic infections, autoimmune diseases, liver disease, and certain inflammatory conditions. Clinical correlation and repeat testing may be needed to determine significance.
Range: Slightly above normal range
