This test is indispensable in clinical oncology for detecting and monitoring monoclonal gammopathies such as multiple myeloma, Waldenström macroglobulinemia, and other plasma cell dyscrasias. A monoclonal spike on serum protein electrophoresis combined with elevated IgG levels warrants further investigation including serum free light chain assay and bone marrow biopsy. In immunodeficiency workups, persistently low IgG levels may indicate primary immunodeficiency or secondary causes such as nephrotic syndrome, protein-losing enteropathy, or immunosuppressive therapy. Serial monitoring of IgG levels helps clinicians assess treatment response and disease progression. I recommend this test for patients presenting with recurrent infections, unexplained fatigue, bone pain, or laboratory findings suggestive of a plasma cell neoplasm. Always interpret IgG levels in conjunction with IgA, IgM, serum protein electrophoresis, and clinical presentation."

Normal IgG levels suggest adequate immune function. No evidence of immunodeficiency, active chronic infection, or monoclonal gammopathy based on this parameter alone.
Low IgG levels may indicate primary immunodeficiency (e.g., common variable immunodeficiency, X-linked agammaglobulinemia), secondary immunodeficiency due to nephrotic syndrome, protein-losing enteropathy, immunosuppressive therapy, or malignancies affecting B-cell function.
