Doctor's Notes

Reviewed by — MBBS, MD (Pathology) · Reg. No. 21521

"Flow cytometry immunophenotyping is central to classifying acute leukemia into T, B, or myeloid lineage. The choice of any six markers should be guided by clinical morphology, blast count, and suspected lineage to avoid diagnostic delay and to plan targeted cytogenetic or molecular testing."

Understanding Your Results

The immunophenotyping profile is interpreted by a pathologist or hematopathologist in conjunction with bone marrow morphology, karyotype, FISH, and molecular results. The antigen expression pattern identifies the lineage and maturation stage of leukemic blasts.
📊

Often positive for B-lineage markers such as CD10, CD19, and TdT.

📊

Often positive for T-lineage markers such as CD3, CD5, or CD7.

📊

Often positive for myeloid markers such as CD13, CD33, or MPO; CD34/HLA-DR may be variable.

📊

Leukaemic blasts may express markers of more than one lineage; correlation with morphology, karyotype, FISH, and PCR is required.

⚠️ When to Consult a Doctor:

Consult an oncologist or hematologist if you have persistent fatigue, unexplained fever, night sweats, weight loss, abnormal bleeding or bruising, enlarged lymph nodes, or if any diagnostic report suggests a hematological malignancy.

Limitations

  • The panel is limited to any 6 selected markers; additional markers may be needed for complete lineage assignment
  • Flow cytometry results must be correlated with morphology, cytogenetics, FISH, and molecular studies
  • Not a standalone diagnostic test; requires hematopathologist interpretation
  • Aberrant antigen expression may produce overlapping lineage features

Risks & Considerations

  • Mild pain or bruising at the venipuncture site
  • Discomfort at the bone marrow aspirate site
  • Rare bleeding or infection at the collection site

Interfering Factors

  • Hemodiluted bone marrow aspirate
  • Clotted specimen
  • Improper storage or transport; refrigeration or freezing
  • Delay in sample transport beyond recommended stability
  • Recent chemotherapy, steroids, or colony-stimulating factors
  • Incomplete or missing Chromosome & FISH analysis requisition form Form 17
