Overview
This test is used to diagnose and monitor a small subgroup of foregut carcinoid tumours that produce predominantly 5-hydroxytryptophan (5-HTP), but very little serotonin and Chromogranin A. Urinary serotonin is measured by LC-MS/MS in a random urine sample. It is not intended as a standalone diagnostic test.

Sample Collection
Strictly avoid bananas, walnuts, tomatoes, pineapples, plums, gooseberries, mirabelles, melons, avocados, aubergines/eggplant, kiwi fruits and drugs such as methocarbamol, mephenesin, guaiphenesin, paracetamol and salicylic acid for at least 48 hours before and during collection. Avoid strenuous exercise. Do not stop prescribed medicines without consulting your doctor.
Method: Random urine collection
Laboratory Analysis
Collect 50 mL (minimum 10 mL) of random urine into a clean container. Immediately add 50% HCl drop by drop until the pH is between 1 and 2. Do not use concentrated HCl. Label the container and store it refrigerated or frozen as advised.
Report Delivery
Keep the specimen refrigerated or frozen until shipment. No special precautions are needed after giving the urine sample. Resume normal diet after collection unless advised otherwise by the laboratory or your doctor.

How to Prepare
- Confirm appointment with DNA Labs India and note collection cut-off times: Wednesday/Saturday by 9 am.
- Avoid listed interfering foods, drugs and strenuous exercise for 48 hours before and during specimen collection.
- Collect a random urine sample in the provided clean container.
- Add 50% HCl drop by drop to bring the pH to 1-2; do not use concentrated HCl.
- Ship or hand over the specimen refrigerated or frozen, and present the patient details to the laboratory.

Limitations
- ⚠Random urine serotonin is not a substitute for 24-hour urine 5-HIAA in classic carcinoid syndrome.
- ⚠No universal reference range exists for random urine serotonin; values must be interpreted by the reporting laboratory and the treating clinician.
- ⚠Serotonin levels can be significantly influenced by diet, drugs and exercise, so adherence to preparation instructions is essential.
- ⚠This test alone cannot diagnose depression, anxiety, or insomnia.
