Overview

This test is used to diagnose and monitor a small subgroup of foregut carcinoid tumours that produce predominantly 5-hydroxytryptophan (5-HTP), but very little serotonin and Chromogranin A. Urinary serotonin is measured by LC-MS/MS in a random urine sample. It is not intended as a standalone diagnostic test.

Sample Collection

Strictly avoid bananas, walnuts, tomatoes, pineapples, plums, gooseberries, mirabelles, melons, avocados, aubergines/eggplant, kiwi fruits and drugs such as methocarbamol, mephenesin, guaiphenesin, paracetamol and salicylic acid for at least 48 hours before and during collection. Avoid strenuous exercise. Do not stop prescribed medicines without consulting your doctor.

Method: Random urine collection

Step 2

Laboratory Analysis

Collect 50 mL (minimum 10 mL) of random urine into a clean container. Immediately add 50% HCl drop by drop until the pH is between 1 and 2. Do not use concentrated HCl. Label the container and store it refrigerated or frozen as advised.

Step 3

Report Delivery

Keep the specimen refrigerated or frozen until shipment. No special precautions are needed after giving the urine sample. Resume normal diet after collection unless advised otherwise by the laboratory or your doctor.

Timeline: Reports are generally available within 2 days. Sample cut-off is Wednesday/Saturday by 9 am.

Patient Instructions

1
Before the Test:No fasting is required. Follow the 48-hour dietary, drug and exercise restrictions as instructed, and do not stop prescribed medicines without your doctor's approval.
2
During the Test:A random urine sample is collected. The lab team will guide you on proper labelling and pH adjustment if the sample is collected at home.
3
After the Test:Return the labelled sample promptly. You may resume normal diet and activities after collection unless advised otherwise.

About This Test

Who Should Get This Test

This test is used to diagnose and monitor a small subgroup of foregut carcinoid tumours that produce predominantly 5-hydroxytryptophan (5-HTP), but very little serotonin and Chromogranin A. Urinary serotonin is measured by LC-MS/MS in a random urine sample. It is not intended as a standalone diagnostic test.

How to Prepare

  • Confirm appointment with DNA Labs India and note collection cut-off times: Wednesday/Saturday by 9 am.
  • Avoid listed interfering foods, drugs and strenuous exercise for 48 hours before and during specimen collection.
  • Collect a random urine sample in the provided clean container.
  • Add 50% HCl drop by drop to bring the pH to 1-2; do not use concentrated HCl.
  • Ship or hand over the specimen refrigerated or frozen, and present the patient details to the laboratory.

Doctor's Notes

Reviewed by — MBBS, MD (Biochemistry) · Reg. No. 21504

"Random urine serotonin should only be interpreted in a clinical context. For suspected foregut carcinoid tumours, I prefer to combine it with 5-HIAA and Chromogranin A rather than relying on any single value."

Understanding Your Results

Interpretation should always be performed by a qualified clinician. This test is not intended to be used alone for diagnosis. Results should be correlated with symptoms, clinical history, imaging and other endocrine markers.
📊

May be seen in foregut carcinoid tumours that produce predominantly 5-HTP; requires further testing and clinical correlation.

Action: Discuss with an endocrinologist; consider 5-HIAA, Chromogranin A and imaging when appropriate.

📊

Does not exclude a serotonin-producing tumour if clinical suspicion remains high.

Action: Follow the treating clinician's advice for additional testing.

📊

May be influenced by diet, medications, or collection factors; not diagnostic alone.

Action: Review pre-test compliance and repeat if clinically indicated.

⚠️ When to Consult a Doctor:

Consult your doctor if you have persistent flushing, diarrhoea, wheezing, rapid heartbeats, severe mood changes, sleep disturbance or any symptoms concerning for a neuroendocrine tumour. Only a doctor can interpret this test in your full medical context.

Limitations

  • Random urine serotonin is not a substitute for 24-hour urine 5-HIAA in classic carcinoid syndrome.
  • No universal reference range exists for random urine serotonin; values must be interpreted by the reporting laboratory and the treating clinician.
  • Serotonin levels can be significantly influenced by diet, drugs and exercise, so adherence to preparation instructions is essential.
  • This test alone cannot diagnose depression, anxiety, or insomnia.

Risks & Considerations

  • No significant medical risks.
  • Urine collection is non-invasive.
  • No radiation or needles are involved.

Interfering Factors

  • Foods: bananas, walnuts, tomatoes, pineapples, plums, gooseberries, mirabelles, melons, avocados, aubergines/eggplant, kiwi fruits.
  • Drugs: methocarbamol, mephenesin, guaiphenesin, paracetamol, salicylic acid.
  • Strenuous exercise for at least 48 hours before and during specimen collection.
  • Improper pH adjustment of urine with 50% HCl or use of concentrated HCl.

Frequently Asked Questions

What is the Serotonin (5-HT) Random Urine Test?
It measures the amount of serotonin (5-hydroxytryptamine) in a random urine sample using LC-MS/MS. It helps healthcare providers evaluate certain serotonin-producing neuroendocrine or foregut carcinoid tumours when interpreted with other tests.
What is the cost of the test?
At DNA Labs India, the test is priced at Rs 6500, and home sample collection is included for online bookings in eligible cities.
Why is this test ordered?
It may be ordered to diagnose or monitor a small subgroup of foregut carcinoid tumours that produce mainly 5-hydroxytryptophan (5-HTP) rather than serotonin. It is not a standalone screening test for depression or anxiety.
Do I need to fast for this test?
No, fasting is not required. However, you must avoid bananas, walnuts, tomatoes, pineapples, plums, gooseberries, mirabelles, melons, avocados, aubergines and kiwi fruits, and certain drugs, for at least 48 hours before and during collection.
What sample is needed?
A 50 mL random urine sample with a minimum of 10 mL is required. It must be collected in a clean container and pH adjusted to 1-2 using 50% HCl drop by drop. Do not use concentrated HCl.
What do the results mean?
Results should be interpreted by an endocrinologist or treating clinician. Elevated levels may prompt further evaluation for foregut carcinoid tumours; normal or low levels do not exclude disease. This test alone is not diagnostic.
How is the sample processed?
The urine sample is analysed using LC-MS/MS, which stands for liquid chromatography-tandem mass spectrometry, a precise analytical method.
What is the turnaround time for reports?
For samples received by Wednesday or Saturday by 9 am, reports are generally available in 2 days. You can access them online, by email, or on WhatsApp.
What foods and medicines should I avoid before the test?
Avoid bananas, walnuts, tomatoes, pineapples, plums, gooseberries, mirabelles, melons, avocados, aubergines/eggplant and kiwi fruits. Also avoid methocarbamol, mephenesin, guaiphenesin, paracetamol, salicylic acid and strenuous exercise for 48 hours before and during collection.
Can I continue my regular medicines?
Do not stop any prescribed medicine without talking to your doctor. Some drugs can interfere with serotonin results; your healthcare provider will tell you what to do.
Is home sample collection available?
Yes, DNA Labs India offers free home sample collection for online bookings of this test in many cities across India. Confirm availability when booking.
Who should I consult after the test?
Discuss the report with the referring doctor or an endocrinologist. They will interpret the results in the context of your symptoms, medical history and other tests such as 5-HIAA or Chromogranin A.
