Sample Collection
No special preparation is required. For gastric lavage collection, overnight fasting is mandatory. Please inform your doctor if you are currently on anti-tubercular therapy.

Laboratory Analysis
A trained healthcare professional will collect the required sample. Sputum must be produced from a deep cough, not saliva. CSF, tissue, bronchial lavage, or bone marrow samples are collected by a qualified physician.
Report Delivery
Samples are labelled, packaged and transported promptly. You can resume normal activities after sample collection unless advised otherwise by your doctor.

Who Should Get This Test
To confirm the diagnosis of tuberculosis by combining smear microscopy, culture with MALDI-TOF identification, molecular detection of M. tuberculosis and rifampicin resistance, and IGRA-based cell-mediated immunity testing.
How to Prepare
- Blood/Bone marrow: Collect 8 mL (5 mL min.) in Myco F/Lytic bottle. Mix by gentle inversion 8-10 times. Ship at 18-22 degree C.
- CSF: Collect 2 mL (1 mL min.) in a red top no-additive tube or sterile screw-capped container. Do not use SST gel barrier tubes. Ship refrigerated.
- Pus/Body fluids (Pleural/Pericardial/Ascitic/Synovial/Ocular) aspirates, semen, BAL, bronchial washings: Submit as much as possible (1 mL min.) in sterile screw-capped container. Ship refrigerated.
- Endometrial curettings/Tissue: Submit in sterile normal saline in sterile screw-capped container. Ship refrigerated.
- Swabs: Submit swabs in 1 mL sterile normal saline in sterile screw-capped container. Ship refrigerated.
- Sputum/Urine: Submit 2 spot (random) morning samples, 5-10 mL (1 mL min.) sputum or 10 mL (5 mL min.) urine in sterile screw-capped container. Do not pool samples. Ship refrigerated.
- Gastric Lavage: Submit 5-10 mL (2 mL min.) gastric lavage in sterile screw-capped container after overnight fasting. Also collect 5 mL (4 mL min.) whole blood in a green top heparin tube. Ship refrigerated. Do not freeze.

Understanding Your Results
AFB Smear: Negative
No acid-fast bacilli seen. This does not exclude tuberculosis; culture and molecular tests are needed.
GeneXpert Ultra: MTB detected; Rifampicin resistance not detected
Mycobacterium tuberculosis complex present and likely susceptible to rifampicin.
GeneXpert Ultra: MTB detected; Rifampicin resistance detected
M. tuberculosis complex present with rifampicin resistance. Further drug susceptibility testing and specialist management are required.
Culture: Growth of M. tuberculosis complex
Active tuberculosis is confirmed. Identification by MALDI-TOF supports the diagnosis.
TB Gold Plus IGRA: Positive
Indicates M. tuberculosis infection or sensitisation. Cannot distinguish latent from active tuberculosis.
Consult your physician if you have a persistent cough lasting more than three weeks, coughing up blood, chest pain, fever, night sweats, unexplained weight loss, or close contact with a person with infectious TB.
Limitations
- ⚠A negative AFB smear or culture does not completely exclude tuberculosis.
- ⚠A positive IGRA cannot differentiate active TB from latent TB infection.
- ⚠Some extrapulmonary forms require invasive sampling procedures.
- ⚠Molecular tests may not distinguish live from dead organisms in treated patients.

- ●Inadequate sample volume or inappropriate storage may decrease bacterial viability.
- ●Cross-contamination during sample processing may cause false-positive results.
