Overview

Elevated values of VMA along with other catecholamine metabolites are suggestive of catecholamine secreting tumor like Neuroblastoma, Pheochromocytoma and other Neural crest tumors. VMA levels may also be useful in monitoring patients who are also treated. Elevated levels are suggestive of Pheochromocytoma but not diagnostic.

Sample Collection

Patient should strictly avoid Theophylline, chocolate, vanilla, banana, alcoholic beverages, tea/coffee, tobacco and strenuous exercise at least 72 hrs before and during specimen collection.

Method: 24-hour urine collection with HCl preservative

Step 2

Laboratory Analysis

Collect all urine passed over a full 24-hour period. Keep the container refrigerated during collection. The container should contain 50% HCl to maintain pH between 1 and 2; do not use concentrated HCl. Record the total 24-hour urine volume and collection times.

Step 3

Report Delivery

Store the sealed container at 2-8°C or frozen if the sample cannot be delivered immediately. Include the recorded 24-hour urine volume on the test form and container. Transport the sample to the laboratory refrigerated or frozen.

Timeline: Reports are delivered within 2 days after the sample reaches the laboratory.

About This Test

Who Should Get This Test

Elevated values of VMA along with other catecholamine metabolites are suggestive of catecholamine secreting tumor like Neuroblastoma, Pheochromocytoma and other Neural crest tumors. VMA levels may also be useful in monitoring patients who are also treated. Elevated levels are suggestive of Pheochromocytoma but not diagnostic.

How to Prepare

  • Avoid the listed substances and strenuous exercise for at least 72 hours before and during collection.
  • Use the collection container provided by the laboratory.
  • Discard the first morning urine and note the start time.
  • Collect all urine passed during the next 24 hours into the container.
  • Store the container in a refrigerator or cool place during collection.
  • Record the total urine volume on the request form and container.
  • Deliver the sample to the laboratory refrigerated or frozen.

Doctor's Notes

Reviewed by — MBBS, MD (Biochemistry) · Reg. No. 21504

"Elevated VMA levels are a useful biochemical clue, but pheochromocytoma and neuroblastoma must be confirmed by imaging and more specific tests like metanephrines. I recommend interpreting VMA results in the full clinical picture."

Understanding Your Results

This test should be interpreted by a physician in the context of clinical presentation and other laboratory findings. One elevated VMA value is not sufficient to make a diagnosis of a tumor.
📊

Normal VMA

No biochemical evidence of excess catecholamine production.

📊

Mildly elevated VMA

Could be due to diet, medication, stress, or collection error; repeat or evaluate with more specific tests.

📊

Moderately to markedly elevated VMA

Suggests possible catecholamine-secreting neuroendocrine tumor; further imaging and metanephrine testing recommended.

⚠️ When to Consult a Doctor:

See an oncologist or endocrinologist if VMA is elevated or if symptoms such as high blood pressure, headache, palpitations, sweating, weight loss, or abdominal pain persist.

Limitations

  • Elevated VMA alone is not diagnostic for pheochromocytoma or neuroblastoma; clinical correlation and imaging are required.
  • Dietary and medication non-compliance may produce false-positive results.
  • Additional studies such as plasma or urinary metanephrines may be needed for confirmation.

Risks & Considerations

  • No significant risk for VMA 24-hour urine collection.
  • Sample leakage or incomplete collection may require repeat testing.

Interfering Factors

  • Theophylline
  • Chocolate
  • Vanilla
  • Banana
  • Alcoholic beverages
  • Tea/coffee
  • Tobacco
  • Strenuous exercise

Frequently Asked Questions

What is the VMA 24-hour urine test?
It measures vanillylmandelic acid (VMA), a breakdown product of catecholamines, in urine collected over 24 hours. It helps detect or monitor neuroblastoma, pheochromocytoma and other neuroendocrine tumors.
How much does the VMA 24-hour urine test cost at DNA Labs India?
The test costs Rs 5000 at DNA Labs India. The price includes free home sample collection in most major cities.
Who should undergo this test?
It is advised for patients with symptoms suggestive of catecholamine-secreting tumours such as persistent or episodic high blood pressure, headaches, palpitations, sweating, weight loss or abdominal pain, and for monitoring known cases.
Do I need to fast for the VMA test?
No, fasting is not required. However, you must avoid certain foods, drinks, medicines and strenuous exercise for at least 72 hours before and during the urine collection.
Why is a 24-hour urine sample needed?
VMA excretion varies throughout the day. A complete 24-hour collection measures the total daily output and is more reliable than a single urine sample.
What should I avoid before this test?
Avoid theophylline, chocolate, vanilla, banana, alcoholic beverages, tea/coffee, tobacco and strenuous exercise for at least 72 hours before and during collection. Do not stop any prescription medicine without consulting your doctor.
Is the VMA test diagnostic for pheochromocytoma?
Elevated VMA is suggestive but not diagnostic on its own. Additional tests such as urinary/plasma metanephrines and imaging (CT or MRI) are needed for confirmation.
How is the test performed?
You collect all urine passed in a 24-hour period in a special container, which contains a preservative. The sample is shipped refrigerated or frozen to the laboratory, where VMA is measured using HPLC-EC.
How long does it take to get the report?
Once the sample reaches the laboratory, the report is usually ready within 2 days. Samples are accepted on Wednesday/Saturday by 9 am as standard schedule.
Can I book a home sample collection?
Yes. DNA Labs India provides free home sample collection for online bookings across multiple cities in India.
Why is acid added to the urine container?
A small amount of 50% hydrochloric acid keeps the urine pH between 1 and 2, which preserves VMA. Do not use concentrated HCl; use only the container provided by the lab.
What does an elevated VMA level mean?
It may indicate excess catecholamine production from a tumour such as neuroblastoma, pheochromocytoma or another neural crest tumour. The result should be interpreted by your doctor in context with other tests.
