Overview

The TORCH panel is requested to evaluate possible infection with Toxoplasma gondii, Rubella virus, Cytomegalovirus, Herpes simplex virus types 1 and 2, Varicella-Zoster virus and Parvovirus B19. It is important in pregnancy because these pathogens can be transmitted across the placenta and cause fetal or neonatal complications. The test is also used in newborn evaluation, bad obstetric history, transplant/donor screening and when clinical features suggest TORCH infection.

Pregnant women with positive or equivocal IgM results, persistent fever, rash or lymphadenopathy, or ultrasound findings suggestive of congenital infection should consult their obstetrician or an infectious disease physician promptly. Newborns with suspected TORCH infection should be evaluated by a paediatrician or neonatologist immediately.

  • ⚠IgM positivity alone does not accurately predict the risk of fetal infection.
  • ⚠Serology may be negative in early infection.
  • ⚠IgG avidity is not included in this panel.
  • ⚠Maternal IgG crosses the placenta, so newborn results require paired maternal-infant interpretation.
