Overview
The purpose of the TORCH Panel IgM Extended test is to detect the presence of IgM antibodies against Toxoplasma gondii, Rubella virus, Cytomegalovirus, Herpes Simplex Virus type 1 and type 2. Detection of these antibodies helps physicians evaluate a suspected acute or recent infection in pregnant women, newborn infants with symptoms of congenital infection, and immunocompromised individuals.

"Positive TORCH IgM results should never be interpreted alone. Clinical context, confirmatory tests such as IgG avidity or PCR, and obstetric or infectious disease consultation are essential, especially in pregnancy."

Test Parameters & Specifications
Sample Stability
- Grossly hemolyzed or severely lipemic specimens
- Improperly labeled sample tubes
- Sample collected in the wrong anticoagulant tube
- Sample delayed beyond the recommended stability period
Understanding Your Results
Reactive IgM may suggest recent toxoplasma infection. Confirm with Toxoplasma IgG seroconversion, IgG avidity, and clinical history. Non-reactive is reported as negative.
Reactive IgM may indicate recent rubella infection or recent vaccination. IgG seroconversion and clinical findings are important for confirmation.
Reactive IgM may suggest primary CMV infection, reactivation, or reinfection. CMV IgG avidity and PCR are useful confirmatory tests.
Reactive IgM suggests recent or recurrent HSV-1 infection. Clinical examination and viral typing may be needed.
Reactive IgM suggests possible HSV-2 infection. Cross-reactivity with HSV-1 can occur, so confirmatory testing is recommended.
If you are pregnant and your TORCH IgM test is positive or equivocal, consult your obstetrician and an infectious disease specialist promptly. Similarly, if a newborn has abnormal TORCH IgM results, urgent pediatric evaluation is required.
Limitations
- ⚠A positive IgM result does not always indicate acute infection; IgM may persist for months
- ⚠A positive IgM result alone does not accurately predict fetal infection
- ⚠Confirmatory testing such as IgG seroconversion, IgG avidity, or PCR is often required
- ⚠False negative IgM results may occur in immunocompromised patients
Risks & Considerations
- ●Minor pain or bruising at the needle site
- ●Dizziness or lightheadedness
- ●Rare risk of infection or excessive bleeding at the puncture site
Interfering Factors
- ●Rheumatoid factor can cause false positive IgM results
- ●Cross-reactivity between HSV-1 and HSV-2 antibodies
- ●Recent blood transfusion or immunoglobulin therapy may interfere with immunoassays
- ●Gross hemolysis, lipemia, or high bilirubin may affect EIA testing
