- Fluorescence In Situ Hybridization (FISH)
Sample Collection
No fasting is required. Bring a duly filled Chromosome & FISH analysis Requisition Form (Form 17) as it is mandatory.
Method: Venipuncture, bone marrow aspiration, or tissue biopsy
Laboratory Analysis
A healthcare professional will collect 4 mL whole blood in a sodium heparin tube or perform bone marrow aspiration or lymph node biopsy depending on clinical indication. The sample must not be frozen.
Report Delivery
Samples are shipped at 18–22°C. Lymph node biopsy in normal saline or formalin-fixed paraffin-embedded tissue block should be shipped at room temperature. Do not freeze.

About This Test
Who Should Get This Test
The test detects the IGH/BCL2 t(14;18) translocation by FISH to confirm the diagnosis of follicular lymphoma and help differentiate it from other B-cell lymphomas.
How to Prepare
- Use sterile technique for bone marrow aspiration.
- Collect 4 mL whole blood in sodium heparin green top tube.
- Invert tube gently 8–10 times; do not centrifuge.
- For tissue, place 5x5 mm lymph node biopsy in normal saline or submit FFPE block.
- Label immediately and ship at specified temperatures; do not freeze.
Doctor's Notes
Reviewed by Dr Pasupathy Arumugam — MBBS, MD (Pathology) · Reg. No. 21521
"FISH for IGH/BCL2 t(14;18) is the standard cytogenetic confirmation for follicular lymphoma. It detects the translocation that drives BCL2 overexpression and helps distinguish follicular lymphoma from reactive lymphoid proliferations and other B-cell lymphomas."

Test Parameters & Specifications
Sample Stability
- Frozen whole blood or bone marrow sample
- Clotted or hemolyzed sample
- Unlabeled or mislabeled specimen
- Missing duly filled Chromosome & FISH analysis Requisition Form (Form 17)
- Insufficient sample volume or inadequate tissue
Understanding Your Results
Consult a medical oncologist or hematologist if you have enlarged lymph nodes, unexplained weight loss, persistent fever, night sweats, fatigue, or other symptoms suggestive of lymphoma.
Limitations
- ⚠A negative result does not completely exclude follicular lymphoma.
- ⚠FISH targets the specific t(14;18) translocation and may miss rare variant translocations.
- ⚠Results must be correlated with histopathology, immunophenotyping, and clinical findings.
Risks & Considerations
- ●Mild pain or bruising at blood collection site
- ●Bone marrow aspiration may cause temporary discomfort
- ●Biopsy site bleeding or infection is rare
Interfering Factors
- ●Low tumor burden below FISH sensitivity
- ●Hemodiluted bone marrow sample
- ●Poorly fixed or necrotic tissue
- ●Delayed transport or inappropriate freezing of sample
