Overview
This assay is used for the diagnosis of peripheral neuropathies such as Guillain-Barré syndrome, primary motor neuron disease, and acute motor and sensory axonal neuropathy. Autoimmune-mediated peripheral neuropathies are associated with the presence of circulating autoantibodies that bind to specific gangliosides. The Ganglioside Antibody Profile IgM Test identifies these antibodies to aid in early and accurate diagnosis, enabling timely therapeutic intervention. It detects IgM antibodies against GM1, GM2, GM3, GD1a, GD1b, GT1b, and GQ1b gangliosides.

Sample Collection
No special preparation or fasting is required. Inform your doctor about any current medications, recent immunoglobulin therapy, or plasmapheresis treatments prior to sample collection.
Method: Venipuncture
Laboratory Analysis
A trained phlebotomist will collect approximately 2 mL of venous blood from a vein in your arm using standard venipuncture technique. The blood is drawn into a Serum Separator Tube (SST).
Report Delivery
Apply gentle pressure on the puncture site with a cotton ball for a few minutes. A small bruise may develop, which is normal. You may resume normal activities immediately after sample collection.

Doctor's Notes
Reviewed by Dr SHAILAJA RAGHUNATH MURDESHWAR — MBBS, MD (General Medicine) · Reg. No. 8052
"Ganglioside antibody testing is a valuable adjunct in the diagnostic workup of suspected autoimmune neuropathies. Anti-GM1 IgM antibodies are frequently seen in multifocal motor neuropathy and certain Guillain-Barré variants, while anti-GQ1b IgM antibodies are highly specific for Miller Fisher syndrome. This panel should always be interpreted alongside clinical presentation, nerve conduction studies, and cerebrospinal fluid analysis. Early detection through this test enables timely initiation of immunotherapy such as intravenous immunoglobulin or plasmapheresis, which can significantly improve patient outcomes. It is important to note that a negative result does not entirely exclude an autoimmune neuropathy, as antibody levels may fluctuate and some conditions may be seronegative."

Test Parameters & Specifications
Sample Stability
- Hemolyzed sample
- Lipemic sample
- Sample collected in incorrect tube type (non-SST)
- Insufficient sample volume (less than 1 mL)
- Sample received at room temperature after 2 hours of collection
Understanding Your Results
Anti-GM1 IgM Positive
Suggestive of multifocal motor neuropathy (MMN) or certain Guillain-Barré syndrome variants. Clinical correlation with nerve conduction studies is recommended.
Anti-GQ1b IgM Positive
Highly specific for Miller Fisher syndrome, characterized by ataxia, ophthalmoplegia, and areflexia. May also be seen in GBS with ophthalmoplegia.
Anti-GD1a IgM Positive
Associated with acute motor axonal neuropathy (AMAN), a pure motor variant of Guillain-Barré syndrome.
Multiple Antibodies Positive
May indicate broader autoimmune neuropathic involvement. Further clinical and electrophysiological evaluation is warranted.
All Antibodies Negative
Does not exclude an autoimmune neuropathy. Some conditions may be seronegative. Clinical correlation and additional investigations are advised.
Consult your neurologist if you experience progressive weakness in one or more limbs, numbness or tingling in the extremities or face, difficulty with balance or coordination, difficulty speaking or swallowing, or changes in vision or hearing. Seek immediate medical attention if symptoms develop rapidly, as conditions like Guillain-Barré syndrome can progress to respiratory failure and require urgent treatment.
Limitations
- ⚠A negative result does not completely exclude an autoimmune neuropathy as some conditions may be seronegative
- ⚠Antibody titers may fluctuate over the course of disease and may decline with treatment
- ⚠Results should always be interpreted in conjunction with clinical presentation, nerve conduction studies, and CSF analysis
- ⚠Cross-reactivity between different ganglioside antibodies is possible and may affect specificity
Risks & Considerations
- ●Minor bruising or pain at the needle insertion site
- ●Slight risk of infection at the puncture site
- ●Rarely, fainting or dizziness during blood draw
Interfering Factors
- ●Recent administration of intravenous immunoglobulin (IVIG) may cause false-positive results
- ●Plasmapheresis performed shortly before testing may lower antibody levels and yield false-negative results
- ●Presence of heterophilic antibodies may interfere with assay results
- ●Hemolyzed or lipemic samples may affect test accuracy

| Comparison | Ganglioside Antibody Profile IgM Test | While the IgM profile detects IgM-class antibodies often associated with acute-phase autoimmune responses, the IgG profile detects IgG-class antibodies that may indicate chronic or past immune activation. Both may be ordered together for comprehensive evaluation. | NCS assesses the electrical conduction of nerves and helps determine whether neuropathy is demyelinating or axonal. The Ganglioside Antibody Profile identifies the autoimmune cause, while NCS characterizes the functional nerve damage pattern. | Anti-MAG antibodies target myelin-associated glycoprotein and are associated with IgM paraproteinemic demyelinating neuropathy. The Ganglioside Antibody Profile covers a broader range of ganglioside targets relevant to different neuropathy subtypes. | CSF analysis in Guillain-Barré syndrome classically shows albuminocytological dissociation (elevated protein with normal cell count). The Ganglioside Antibody Profile provides immunological specificity that CSF analysis cannot. |
