Method: Venipuncture
Laboratory Analysis
A blood sample of approximately 2 mL will be drawn from a vein in your arm using standard venipuncture technique. The procedure typically takes 3 to 5 minutes.
Report Delivery
After blood collection, slight bruising or soreness at the puncture site is normal and resolves within a day. You may resume normal activities immediately after sample collection.

How to Prepare
- No fasting is required prior to sample collection
- Stay well hydrated before the blood draw
- Inform the phlebotomist if you have a history of difficult blood draws or bleeding disorders
- Wear short sleeves or clothing with loose sleeves for easy access to the arm
- Avoid strenuous exercise immediately before sample collection

"Free light chain assay is an indispensable tool in the diagnostic workup and monitoring of plasma cell neoplasms. When combined with serum protein electrophoresis and immunofixation electrophoresis, it achieves approximately 99% sensitivity for detecting plasma cell disorders. The kappa/lambda ratio is particularly valuable for identifying clonal excess even in early-stage disease. I recommend this test as a baseline for all patients with suspected multiple myeloma, monoclonal gammopathy of undetermined significance (MGUS), or AL amyloidosis, as well as for quantitative monitoring of patients with oligosecretory disease where traditional urine assessments may be inadequate."

Understanding Your Results
Elevated Kappa Free Light Chains with abnormal Kappa/Lambda ratio (above 1.65)
May indicate a kappa-restricted clonal plasma cell disorder such as multiple myeloma, MGUS, or AL amyloidosis. Further evaluation with bone marrow biopsy, SPEP, and IFE is recommended.
Elevated Lambda Free Light Chains with abnormal Kappa/Lambda ratio (below 0.26)
May indicate a lambda-restricted clonal plasma cell disorder. Lambda light chain disease has been associated with a higher risk of renal damage and AL amyloidosis. Confirmatory testing is warranted.
Both Kappa and Lambda Free Light Chains elevated with normal ratio
May indicate polyclonal activation rather than a clonal disorder. This pattern can be seen in autoimmune conditions, chronic infections, liver disease, or renal impairment. Clinical correlation is essential.
Normal Free Light Chains with normal Kappa/Lambda ratio
A normal result significantly reduces the likelihood of an active clonal plasma cell disorder. However, it does not completely exclude early-stage disease in the presence of strong clinical suspicion.
Consult your doctor if your free light chain levels or kappa/lambda ratio are outside the normal reference range. Also seek medical advice if you are experiencing persistent bone pain, unexplained fatigue, recurrent infections, weight loss, or shortness of breath, as these may be symptoms of an underlying plasma cell disorder. Any abnormal test result should be evaluated in the context of your complete clinical picture by a qualified oncologist or hematologist.
Limitations
- ⚠This test alone cannot confirm a diagnosis of multiple myeloma and must be interpreted alongside other laboratory and clinical findings
- ⚠Results may be unreliable in patients with severe renal impairment as reduced clearance elevates free light chain levels independent of plasma cell neoplasia
- ⚠Antigen excess (hook effect) may occur in patients with very high free light chain concentrations, potentially yielding falsely low results unless dilution protocols are followed
- ⚠This test measures free light chains only and does not detect light chains bound to intact immunoglobulins

Interfering Factors

- ●Extremely high levels of free light chains (>1000 mg/L) may require sample dilution and could affect accuracy
- ●Lipemic, icteric, or hemolyzed samples may interfere with nephelometric measurements
- ●Renal impairment can independently elevate free light chain levels due to reduced clearance
