When combined with serum protein electrophoresis and immunofixation electrophoresis, it achieves approximately 99% sensitivity for detecting plasma cell disorders. The kappa/lambda ratio is particularly valuable for identifying clonal excess even in early-stage disease. I recommend this test as a baseline for all patients with suspected multiple myeloma, monoclonal gammopathy of undetermined significance (MGUS), or AL amyloidosis, as well as for quantitative monitoring of patients with oligosecretory disease where traditional urine assessments may be inadequate."
