Overview
The purpose of the AML PCR Panel is to detect specific genetic mutations (fusion genes) that are characteristic of certain AML subtypes. This aids in confirming the diagnosis, determining the prognosis, selecting appropriate targeted therapy, and monitoring treatment response. For example, PML/RARA positivity indicates Acute Promyelocytic Leukemia (APL), which is treated with ATRA and arsenic trioxide. Detection of AML/ETO and inv(16) identifies core-binding factor leukemias, which have a favorable prognosis and may benefit from high-dose cytarabine. The test is also used for minimal residual disease (MRD) monitoring after chemotherapy or stem cell transplantation.

Sample Collection
No special preparation is required. A doctor's prescription is mandatory. Inform your physician about any ongoing medications or recent blood transfusions.
Method: Venipuncture or bone marrow aspiration
Laboratory Analysis
Peripheral blood will be drawn from a vein in your arm. For bone marrow, a small sample will be taken from the hip bone under local anesthesia. The procedure is quick and generally well-tolerated.
Report Delivery
You can resume normal activities immediately. If bone marrow aspiration was performed, you may experience mild soreness at the site, which usually resolves within a few days. Apply ice if needed and avoid strenuous activity for 24 hours.
Timeline: Reports are typically available within 5-6 days after the sample reaches the laboratory.

- Incorrect container (e.g., heparin instead of EDTA)
- Sample received after 24 hours without proper storage
- Unlabeled or mislabeled sample
Understanding Your Results
AML/ETO positive
Indicates t(8;21) translocation, associated with AML with maturation (FAB M2). Generally favorable prognosis, responsive to high-dose cytarabine.
inv(16) positive
Indicates inversion of chromosome 16, associated with AML with abnormal eosinophils (FAB M4Eo). Favorable prognosis, responsive to cytarabine-based therapy.
PML/RARA positive
Indicates t(15;17) translocation, diagnostic of Acute Promyelocytic Leukemia (APL). Highly responsive to ATRA and arsenic trioxide; medical emergency.
All negative
No evidence of the three common fusion genes. Other genetic tests may be needed to identify alternative mutations. Does not exclude AML.
Consult your oncologist if you experience symptoms such as persistent fever, fatigue, unexplained bruising or bleeding, shortness of breath, weight loss, or swollen lymph nodes. Early diagnosis and treatment are crucial for AML.
Risks & Considerations
- ●Minimal risk of bleeding or bruising at the blood draw site
- ●Rare risk of infection at the bone marrow aspiration site
- ●Discomfort during bone marrow aspiration
Interfering Factors
- ●Hemolysis or clotting of the sample
- ●Inadequate sample volume
- ●Delay in transport leading to RNA degradation
- ●Contamination with non-leukemic cells
- ●Recent blood transfusion (may dilute leukemic cells)

|---|---|---|---|---|
| Comparison | AML PCR Panel (AML/ETO, inv(16), PML/RARA) | Karyotyping visualizes entire chromosomes and can detect large structural abnormalities, but requires dividing cells and has lower resolution. PCR is more sensitive for specific fusion genes and can detect submicroscopic changes. | FISH uses fluorescent probes to detect specific DNA sequences and can be performed on non-dividing cells. PCR is more sensitive for minimal residual disease detection. | NGS can detect multiple mutations simultaneously, including point mutations and indels. PCR is faster and more cost-effective for targeted fusion gene detection. |
