Strongly associated with Granulomatosis with Polyangiitis (GPA), formerly known as Wegener's granulomatosis. This corresponds to the cytoplasmic ANCA (c-ANCA) pattern on immunofluorescence. Clinical features may include upper and lower respiratory tract involvement and glomerulonephritis.
Associated with Microscopic Polyangiitis (MPA) and Eosinophilic Granulomatosis with Polyangiitis (EGPA). This corresponds to the perinuclear ANCA (p-ANCA) pattern on immunofluorescence. Clinical features may include rapidly progressive glomerulonephritis and pulmonary capillaritis.
May be seen in drug-induced vasculitis, inflammatory bowel disease, autoimmune hepatitis, cystic fibrosis, and other autoimmune conditions. These atypical ANCA antibodies require careful clinical correlation and are not specific to primary vasculitis.
ANCA-associated vasculitis is less likely but cannot be entirely excluded. A small percentage of patients with GPA, MPA, or EGPA may be ANCA-negative. Clinical suspicion should prompt further investigation including imaging and biopsy if indicated.
