Sample Collection
Overnight fasting is preferred. Ensure the Coagulation Requisition Form (Form 15) is duly filled and available at the time of sample collection. Inform the phlebotomist about any anticoagulant medications you are currently taking. Avoid strenuous physical activity before the test.
Method: Venipuncture
Laboratory Analysis
A venipuncture will be performed, typically from a vein in the arm. Approximately 3 mL of blood will be collected into a Blue Top (Sodium Citrate) tube. The tube must be mixed thoroughly by gentle inversion 3-4 times immediately after collection to prevent clotting within the tube.
Report Delivery
Apply gentle pressure on the puncture site with a cotton ball or bandage for a few minutes to prevent bruising. There are no significant restrictions after sample collection. You may resume normal activities immediately.

How to Prepare
- Overnight fasting is preferred for accurate results
- A duly filled Coagulation Requisition Form (Form 15) is mandatory
- 3 mL whole blood collected in 1 Blue Top (Sodium Citrate) tube
- Mix the sample thoroughly by gentle inversion immediately after collection
- Transport the sample to the lab within 4 hours of collection
- If transport within 4 hours is not possible, prepare Platelet Poor Plasma (PPP) within 1 hour by centrifuging at 3600 rpm for 15 minutes
- Transfer supernatant to a clean plastic tube and centrifuge again at 3600 rpm for 15 minutes
- Transfer the final supernatant (PPP) to a labelled clean plastic screw-capped vial and freeze immediately
- Ship frozen. Do not thaw the sample during transport

"D-Dimer quantitative testing is a valuable first-line investigation when thromboembolic disease is suspected. A negative D-Dimer result, combined with a low clinical pre-test probability, can effectively rule out deep vein thrombosis and pulmonary embolism without the need for imaging studies. However, elevated D-Dimer levels are not specific to thrombosis alone and can be raised in various conditions including infection, malignancy, pregnancy, liver disease, and post-surgical states. Therefore, a positive D-Dimer result should always be interpreted in conjunction with the clinical context and followed up with appropriate imaging such as Doppler ultrasonography or CT pulmonary angiography. This test is also valuable for monitoring patients on anticoagulant therapy."

Understanding Your Results
A D-Dimer level below the cutoff value (typically < 0.5 mg/L FEU) is considered normal or negative. In patients with a low-to-moderate clinical pre-test probability for DVT or PE, a negative D-Dimer result is highly useful in effectively ruling out acute thromboembolic disease. Further imaging studies are generally not required in this scenario.
Low likelihood of acute thromboembolism. Useful for ruling out DVT and PE in low-risk patients.
An elevated D-Dimer level above the cutoff value indicates increased fibrin degradation activity in the blood, which may suggest the presence of an active blood clot. However, elevated D-Dimer is not specific to thromboembolism and can be seen in many other conditions. A positive result warrants further diagnostic evaluation, including imaging studies such as Doppler ultrasonography for DVT or CT pulmonary angiography for PE.
Suggests possible thromboembolic event. Requires further investigation with imaging. May also be elevated in infection, inflammation, pregnancy, malignancy, liver disease, and post-operative states.
