- Submit formalin fixed paraffin embedded tissue block with at least 10% tumor tissue.
- Alternatively, provide 4 mL whole blood or 4 mL bone marrow in a Green Top (Sodium Heparin) tube.
- Ship tissue blocks at room temperature; ship blood/bone marrow at 18-22°C.
- Do not freeze the samples under any circumstances.
- Include the duly filled Chromosome & FISH analysis Requisition Form (Form 17) with the sample.
