Overview
This panel helps to assess if a patient is likely to develop complications like a cytokine storm and coagulation defects due to SARS-CoV-2 infection. It combines inflammatory markers, organ function tests, cardiac markers, and coagulation parameters for risk stratification in COVID-19.
- Test Code
- 3507
- Price
- ₹16,380
- Sample Type
- Serum, EDTA plasma, EDTA whole blood, sodium citrate plasma/whole blood, and nasopharyngeal/oropharyngeal swab in viral transport medium
- Result Time
- Sample daily by 11 am; report 2 days.
- Fasting Required
- No
- Method
- Real Time RT-PCR, Electrical Impedance, VCS, Spectrophotometry, Indirect ISE, Immunoturbidimetry, Photo-optical clot detection, ECLIA, CLIA, IFCC, CMIA
Sample Collection
Duly filled Covid-19 Clinical Information Form (Form 44) / SRF ID as mandated by GOI is mandatory AND Duly filled Coagulation Requisition Form (Form 15) is mandatory. Patient should avoid Biotin at least 8 hours prior to specimen collection. Overnight fasting is preferred.
Method: Venepuncture and nasopharyngeal/oropharyngeal swab
Laboratory Analysis
A trained phlebotomist will collect blood samples from a vein and nasopharyngeal/oropharyngeal swabs using appropriate infection control precautions.
Report Delivery
Apply gentle pressure at the venepuncture site to prevent bruising. Samples are processed and transported at specified temperatures.
Timeline: Sample daily by 11 am; report 2 days.

Who Should Get This Test
This panel helps to assess if a patient is likely to develop complications like a cytokine storm and coagulation defects due to SARS-CoV-2 infection. It combines inflammatory markers, organ function tests, cardiac markers, and coagulation parameters for risk stratification in COVID-19.
How to Prepare
- Carry duly filled Covid-19 Clinical Information Form (Form 44) / SRF ID.
- Carry duly filled Coagulation Requisition Form (Form 15).
- Avoid biotin supplements at least 8 hours before collection.
- Overnight fasting is preferred.
- Collect serum from two SST tubes and separate EDTA plasma within 2 hours.
- Mix blue top sodium citrate tubes thoroughly by inversion.
- Transport sodium citrate whole blood to the lab within 4 hours or prepare platelet-poor plasma and freeze immediately.
- Collect nasopharyngeal and oropharyngeal swabs in viral transport medium and ship refrigerated; do not freeze.
Doctor's Notes
Reviewed by Dr Ramarao Paidisetty — MBBS, MD (Biochemistry) · Reg. No. 21504
"This panel is useful for risk stratification in SARS-CoV-2 positive patients with moderate to severe disease. Elevated IL-6, CRP, ferritin, and D-dimer may indicate a hyperinflammatory state and can guide timely escalation or immunomodulatory therapy. Results should always be interpreted with clinical status and current guidelines."

Test Parameters & Specifications
Sample Stability
- Insufficient sample volume
- Sample collected in wrong tube
- Frozen whole blood where refrigeration is specified
- Thawed frozen sample where frozen shipping is required
- Grossly haemolysed or lipemic sample
- Missing mandatory requisition forms or SRF ID
Understanding Your Results
Detected indicates presence of viral RNA; Not Detected does not rule out infection if sampling or timing is suboptimal.
Elevated levels can occur in cytokine release syndrome and severe COVID-19; higher levels may indicate greater inflammatory burden.
Elevated values suggest an acute inflammatory or hyperinflammatory response; very high ferritin may be seen in cytokine storm.
Elevated D-dimer or prolonged PT may indicate coagulation activation and increased thrombotic risk.
Elevated values may suggest myocardial injury or cardiac stress associated with severe COVID-19.
Abnormal liver or kidney function tests may indicate organ involvement and influence treatment decisions.
If you develop fever, cough, shortness of breath, persistent chest pain, confusion, or bluish lips or face, seek emergency medical care immediately.
Limitations
- ⚠This panel is not a standalone diagnostic test and should be interpreted with clinical findings.
- ⚠It may not predict all complications in every patient.
- ⚠Results can vary with time of testing, co-existing infections, and treatment already received.
- ⚠Reference ranges may vary by laboratory, age, sex, and clinical context.
Risks & Considerations
- ●Mild pain or bruising at venepuncture site
- ●Transient discomfort during swab collection
- ●Rare risk of infection at needle site
- ●Vasovagal reaction such as dizziness
Interfering Factors
- ●Recent biotin intake
- ●Haemolysis
- ●Lipemia
- ●Improper sample handling or delayed transport
- ●Anticoagulant therapy may alter coagulation results
